Decoding the Family Model Program:

Looking for care that doesn’t follow the usual script? That’s exactly what the Family Model Program is designed for! Whether it’s physical, intellectual, or developmental disabilities, this program is crafted to meet a variety of needs. It’s about creating a comfortable environment that feels like home, because it is home!  We match individuals with a supportive single-family residence called a Family Model Home.  We’re offering genuine support with community involvement and opportunities for employment, if that is your goal!  You can reach for the stars, and we’re here to help!

Are You Eligible:

To kick off this journey, you’ll need to be enrolled in the DIDD, CHOICES, or ECF Program through your insurance provider. We’re here for those assessed as Level 3 or 4 in terms of their required level of care, ready to support a diverse range of disabilities and needs. But hey, if you’re in need of nursing staff or assistance with injectable medications, we might not be the perfect match.
