Our Family Model Services
AdvanceCare's Family Model Services offers care to individuals with intellectual and developmental disabilities. This could include ADHD, autism, or behavioral disorders. Some individuals may be struggling with Down Syndrome, mobility issues, dementia, or other impairments. These services embrace three programs: DDA, ECF, and CLS Choices. Read below to get more information!
DDA: Department of Disability and Aging
Previously called DIDD, DDA includes assistance with daily living skills, transportation, respite care, and ensuring a healthy and happy life for the supported individual. At AdvanceCare, we realize that one of the biggest triumphs for families with a loved one who has an intellectual or developmental disability is making sure they will be receiving the same love and care they would receive from a family member. Our services stimulate creativity, promote growth, and enhance independence.
ECF: Employment Community First
Our ECF program is for people who have an intellectual or developmental disability. We provide residential services and explore job opportunities for individuals who want to work and assist them through every phase of the employment process. We help them choose positions that match their skills and preferences. This program will allow your loved ones to grow in their communities through supported employment and give them the tools and resources they need to successfully fulfill their dreams of living a more independent and fulfilling lifestyle.
CLS (Choices): Community Living Supports
CLS Choices is a community-based service for seniors with disabilities. It encompasses a continuum of support options in which we pair the individual with a family model professional who will integrate the individual into the family experience and provide services that support each resident’s independence and integration into the community. Our services are based on each individual’s centered plan of care. Services may include hands-on assistance with daily living skills, supervision, and transportation.
AdvanceCare's Family Model Professionals
Family Model Professionals (FMPs) provide a family environment for clients who struggle with disabilities. By opening up their home, an FMP gives care and assistance to individuals who need support. Jodie B, an AdvanceCare FMP, says, "“One individual in my care has physical challenges... Recently, she helped herself in a situation that previously was a great challenge. We celebrated together as she realized she has overcome another obstacle.” Our FMPs touch lives everyday!

What Services are Included in AdvanceCare's Family Model Services Program?
For many people, losing the ability to do simple daily things can be frustrating. That’s where AdvanceCare comes in! Our Family Model Professionals are there to support you with daily living skills so that you can have a better quality of life. Care plans can include some, if not all, of the following:
- Bathing, dressing, grooming, and hygiene
- Mobility assistance
- Developing communication skills and support to communicate their choices
- Behavior monitoring and cognitive support
- Special diets and eating assistance
- Scheduling and managing appointments
- Learning new skills while reaching personal and therapy goals
- Assistance to reach their full potential through the use of technology
- Educate and support individuals to understand and exercise their fundamental rights
- Maintain and build family and community connections
- Educate individuals to feel safe at home and in the community
- Assist with budgeting and understanding the impact of employment and benefits
- Creating a resume and developing interview skills
- Job searching and applying for employment
- Assisting individuals to meet their personal and therapy goals

Family Model Services FAQs
At AdvanceCare, we provide more than services; we offer a new way of life. Whether individuals aim to become a part of the community, find employment, or simply enjoy life, we empower them to reach their goals. Our services go beyond help with daily tasks or companionship (although we offer those services, too); we represent a new way to live. AdvanceCare is not a chain. Our Family Model Services touch lives!
If you choose our Family Model Services, first, you will meet with potential Family Model Professionals to determine compatibility. When you find a strong match, you will transition into the FMP’s home, receiving continuous support in a stable environment.
Ongoing support and monitoring will occur by the Program Manager and our Medical Team, who will conduct home visits and oversee medication management and doctor’s visits.

You can regain your independence while learning new skills in a home environment! You’ll receive personal care assistance while developing relationships and exploring job opportunities. You will not only become a part of a family, but you’ll integrate into the community, as well. At AdvanceCare, we touch lives!
Our services offer care to individuals with intellectual and developmental disabilities. This could include ADHD, autism, or behavioral disorders. Some individuals may struggle with Down Syndrome, dementia, or other impairments. AdvanceCare helps people with different needs and disabilities; this is not a one-size-fits-all approach.
Local Area Agencies on Aging and Disability provide intake for many of the home and community based services programs for older adults and adults with physical disabilities. You can call the TENNOPT line at 1-866-836-6678.
To apply for the Employment and Community First Program,
- Individuals can apply on their own by completing the online Self-Referral Form. Be sure your web browser (used to access information on the web or internet) is Internet Explorer versions 8, 9, or 10, Google Chrome, or Firefox. TennCare’s Privacy Policies can be found here: Privacy Policies.
- A list of possible information that may be needed to complete the financial eligibility part of the application can be found here: ECF CHOICES application items for financial eligibility.
- Do you need help to complete the self-referral? We can help you fill out the form OR we can fill out the form for you with the facts you give us.
- If you have TennCare and your health plan is Wellpoint (formerly known as Amerigroup), BlueCare, or United Health Care Community Plan, you can call them for free. Tell them you need help with a self-referral for Employment and Community First CHOICES. The number is on your TennCare card. The toll-free numbers are also listed below:

To apply for Choices programs,
- The Area Agencies on Aging and Disability (AAAD) can assist applicants not currently enrolled in TennCare in the application process. Additionally, the AAADs serve as a resource for information and education regarding CHOICES and other long-term services and supports.
- If you are interested in applying for CHOICES, contact your local AAAD: Area Agency on Aging and Disability (AAAD) TN Map (Use this map to view AAAD’s by County).
- Call 1-866-836-6678 from anywhere in the state to be automatically directed to your nearest Area Agency.
