Dianne lived in (what is referred to as) supported living and did not get along well with her two roommates. Dianne has diabetes, and due to frequent staff changes, she was unable to follow a healthy eating plan. At her worst point, Dianne weighed over 300 lbs., and her doctor was about to put her on insulin to control her diabetes.
Dianne’s case manager reached out to an AdvanceCare employee about moving her into a family model home, which is a residential service that AdvanceCare provides to individuals who need safety and support.
Dianne was thrilled about being in a family environment and receiving personalized care. After living with her AdvanceCare FMP for one year, Dianne lost 50 lbs. Her FMP encourages her to be healthy and independent, and Dianne is very happy with her progress.
Brittney bounced around in different supported living programs for over a decade. By the age of 34, Brittney was struggling with things like hygiene and meal preparation, and she was frustrated with inconsistent staffing in her supported living environments.
One of Brittney’s friends introduced her to the AdvanceCare Family Model Professional program. Brittney, craving a stable family environment, contacted her case manager to make the switch.
Brittney’s FMP has helped her cope with her feelings and process the consequences of her behavior. She has learned how to dress appropriately and how to apply make-up. Brittney’s daily living and social skills have improved tremendously, and she feels comfortable speaking to her AdvanceCare program manager to ensure that her needs are being met.
